Although the Silent Hill series often revolves around the eerie, fog-laden American resort town, Silent Hill f breaks this mold by setting its story in 1960s Japan. This shift in location reflects the series' tradition of experimenting with standalone narratives, as seen in Silent Hill 2, and parts of Silent Hill 4: The Room and Homecoming, which also ventured beyond the town's limits. Konami's confirmation reassures players that diving into the world of Silent Hill f requires no prior knowledge of the 26-year-old franchise.

The protagonist of Silent Hill f is Shimizu Hinako, a teenager grappling with societal and familial expectations. The story, penned by Ryukishi07, known for the When They Cry visual novel series, promises a deep and engaging narrative. The Japanese-language reveal trailer from March highlighted that Silent Hill f is set to be the first game in the series to receive an 18+ rating certification in Japan, signifying its mature content.

As the game remains in development, its rating could still change. However, past entries like Silent Hill, Silent Hill 2, Silent Hill 3, and Silent Hill: The Room have typically been rated CERO:C for audiences aged 15 and above, while other international releases varied between CERO:C and CERO:D (ages 17+). Silent Hill f is poised to carry a Mature rating in the U.S., PEGI 18 in Europe, and CERO:Z in Japan, reflecting its adult-oriented themes.
